Timely Testing to detect the rate of kidney failure due to diabetes

The overall goal is to increase the adoption of evidencebased standards to identify CKD in Medicare patients through an annual urinary microalbumin measurement for individuals with diabetes. The primary target groups for this intervention are primary care physicians (PCPs) and other practitioners who care for individuals with diabetes.

Slowing the progression of kidney disease in hypertensive individuals with diabetes through the use of an ACE inhibitor and/or an ARB agent

The overall goal is to increase the frequency with which individuals with diabetes and earlier stage CKD (stage 1-4) are treated with an ACE inhibitor and/or ARB agent for the treatment of hypertension. The primary target groups for this intervention are PCPs and other practitioners who care for individuals with diabetes. FMQAI will work with partners to implement system changes, such as incorporation of clinical standards in health information systems by physician practices.

 AV fistula placement and maturation as a first choice for arteriovenous access where medically appropriate for individuals who elect, as part of timely renal placement therapy counseling, hemodialysis as their treatment option for kidney failure.

The overall goal is to ensure that Medicare patients receive comprehensive renal placement therapy counseling (including discussion of all available treatment options) timely enough to allow sufficient time for AV fistula evaluation, placement, where appropriate, and maturation to occur i f hemodialysis is the treatment choice for kidney failure. An AV fistula is the “gold standard” for obtaining vascular access for hemodialysis and should be considered as a first option for all medically appropriate individuals who elect hemodialysis for the treatment of kidney failure.
