Safety Culture

Under CMS directives, implementation of the Hospital Leadership and Quality Assessment Tool (HLQAT) and the AHRQ Hospital Survey on Patient Safety Culture (HSPSC) are required for the Hospitals participating in 9th SOW Patient Safety activities.

Nursing Homes participating in the 9th SOW Patient Safety activities are only required to implement the AHRQ Nursing Home Survey on Patient Safety Culture.

FMQAI will provide expertise, training, and resources related to the administration, methodology, and processes for these tools. The information and experience that hospitals and nursing homes gain from the administration of these tools will transfer to future quality improvement activities. The surveys are just part of the several facets of the projects, all geared toward helping a provider determine where on a continuum their organization stands regarding leadership efforts that foster changes for improved quality of care and patient safety.
