Learning and Action Networks (LANs)

FMQAI will strive to achieve transformational change in selected physician offices through a Cardiac Learning and Action Network (LAN) and individual interventions. FMQAI will collaborate with the Department of Health and Human Services, the Office of the National Coordinator, the Centers for Disease Control, and other stakeholders to address the management of patients with coronary artery disease or peripheral vascular disease.

Focusing on cardiovascular disease, FMQAI will work with healthcare professionals to address the management of Medicare patients through the following measures:

  • Blood Pressure Control
    • Patients with Coronary Artery Disease or Ischemic Vascular Disease whose most recent blood pressure during a measurement year is less than 140/90 mm Hg
  • Lipids Management
    • Patients with Ischemic Vascular disease whose most recent low-density cholesterol screening (LDL-C) is less than 100
  • Aspirin Use
    • Patients with Ischemic Vascular Disease who have documentation of use of aspirin or other antithrombotic medication during a 12-month measurement period
  • Smoking Cessation
    • Smoking cessation counseling for patients who smoke

FMQAI will partner with the four Florida Regional Extension Centers (RECs) to facilitate a LAN for REC-recruited practices after practices implement their electronic health record (EHR) system. The LAN will focus on the practice’s effective use of their EHR for clinical decision support, clinical quality improvement, and tracking and improving cardiac population health.

FMQAI will assist practices with:
  • Benchmarking
  • Using the registry and care management functions of their EHR
  • Interpreting EHR data and reporting to identify and address disparities in care
  • Educational programming, best practices, and intervention tools to improve use of preventive services and cardiac health, including approaches to promote engagement of the patient and family to improve patient health and self-management
  • Identifying and sharing best practices and success stories related to the effective use of HIT / Meaningful Use / HIE to improve use of preventive services and cardiac population health
  • Sharing effective strategies for patient self-management
  • Using resources provided through the LAN
  • Demonstrating Meaningful Use to participate in the CMS EHR Incentive Program
