Every Diabetic Counts (EDC) is a pilot project of FMQAI the Medicare Quality Improvement Organization for Florida, administered under contract with the Centers for Medicare and Medicaid Services (CMS). Through community partnerships, EDC will utilize culturally appropriate and community-based diabetes self-management education to improve diabetes self-management behaviors and health outcomes of selected African American and Hispanic persons with diabetes in Florida.
