FMQAI will recognize hospitals that have participated in Centers for Medicare and Medicaid Services quality improvement projects, have established infrastructures for continuous quality improvement, and have instituted and maintained transformational systems changes.

Certification is awarded to QIO collaborating hospitals that meet all of the pre-established FMQAI quality improvement criteria.

  • Certification criteria includes the following:
    • Publicly report the full hospital measure set (22 measures) and submit discharges from the 4th quarter 2005 discharges to the CMS Clinical Warehouse by May 15, 2006
    • Achieve 80% validity of hospital abstracted records by the CDAC for 4th quarter 2005
    • Establish a multi-disciplinary team for all of the selected clinical topics (hospitals can select from AMI, HF, Pneumonia and SCIP-Inf)
      • include a physician champion for each selected topic
      • demonstrate governing board involvement
    • Submit one process improvement report by May 15, 2006 for at least one clinical topic
      • include data from 1st quarter 2005
