If you are in the hospital and have been told you are ready to go home (discharge) and you do not agree, you have the right to have a “second opinion.” FMQAI can provide the “second opinion”.

To file a hospital appeal call 866-800-8754
Sunday through Saturday between 8:00 am and 4:30 pm including holidays. Calls received after 4:30pm will be returned the next day. You do not need to leave the hospital until FMQAI calls you with the results of the “second opinion.”

If you are in the skilled nursing facility, have home health care, hospice or comprehensive outpatient rehabilitation and you are told that Medicare will not pay anymore for your skilled services, you can ask for a “second opinion.”
